Report An Accessibility Barrier
The University of California, Santa Cruz is committed to equal access and inclusion for people with disabilities and strives to have a barrier-free environment.
Purpose of the Report An Accessibility Barrier form
Use the form to report an accessibility barrier that prevented you or someone else from fully taking part in a university program, service, or activity. You can also use this form to make a more general suggestion on how we can improve accessibility at UCSC.
Your report will be sent to and reviewed by an ADA Access Coordinator who will act, as appropriate, in a timely fashion. Please note: In order to address the barrier, your response may be shared with technical and subject-matter experts.
